Sample Performance Review

Below is a sample performance evaluation form. Performance reviews and evaluations are used to highlight an employee’s strengths and successes. They are also used to identify areas in which the employee could improve. Many companies utilize performance reviews for these purposes alone, but such evaluations can also be used to demonstrate to USCIS that an employer-employee relationship exists. The format below has not been endorsed by USCIS, but is merely an example of the kind of information that should be included. All persons, organizations, and activities mentioned below are fictional. Other formats may be better suited to the individual petitioner’s needs.

In this instance, the evaluation is conducted by using a letter grading system. Each letter grade represents a score out of 20 points. The total score is tallied at the end of the performance review to produce an overall evaluation of the employee’s work.

This evaluation is performed based on the following criteria: (a) Communication Skills; (b) Motivation; (c) Interpersonal Skills; (d) Decision Making; (e) Knowledge of Work; and (f)  Management.

  • Reason/Comments: Mr. Mohan frequently exceeds in time management and implements his project duties exceptionally.
